The rule

Adapt N: if this creature has no +1/+1 counters on it, put N +1/+1 counters on it.

In play

You pay for a bigger creature, but the ability does nothing while the counters are there. Several cards react when they arrive: card draw (Fetid Gargantua), Spawn (Basking Broodscale), returning a permanent from the graveyard (Evolution Witness).
